First Visit Paperwork
If you do not have access to a computer or do not feel comfortable using a computer, you will be sent a packet of information and forms which you will need to complete a bring with you to the appointment. If you are handwriting these are bring them with you, we ask that you plan to arrive at least 30 minutes before your first visit to allow our staff to collect, confirm and input your information.
If you are comfortable using a computer, you may click HERE to complete all necessary forms and submit them to us electronically. Also, you will be able to read all of the pre-appointment information we would normally send you. If you complete the forms on-line, you only need to report 10 minutes before your scheduled appointment.
